Patient
Care
Treatment is individualized and tailored to a couple’s
clinical needs. The couples are encouraged to have
an active involvement during our care. We aim to
provide a high standard of patient care throughout
our patient’s time with us.
Treatment
Overview
Approximately 15% of women have difficulty in
conceiving. As there are many reasons for infertility,
it is important that a unit offers a comprehensive
service. We believe the complex procedures such
as IVF and ICSI shouldn’t be contemplated
unless the simple treatments are proved to be
inappropriate. All treatments involving the use
of ovarian stimulation drugs are closely monitored
by blood test and ultrasound.

Treatment
of Ovulation Disorders
Assistance may be given to couples trying to plan
their family by monitoring menstrual cycles and
timing natural intercourse. If ovulation is not
regular then drugs may be used in the early part
of the cycle to help ovulation be more reliable.
To optimize the chances of conception, an ovarian
stimulation protocol is followed. The drugs used
mimic and regulate natural processes and aid growth
of follicles (fluid-filled sacs which contain
the egg) and bring about ovulation. Your individual
protocol and drugs will be discussed with you
at consultation.
Intra-Uterine
Insemination
Intra-Uterine insemination procedure we carry
out usually involves the use of drugs for follicular
stimulation and ovulation induction. When ovulation
has occurred, a sperm sample is carefully washed
as for IVF treatment and a portion of it is placed
high into uterus. Intra-uterine insemination is
only used if one or both of the fallopian tubes
are known to be open.

In-Vitro
Fertilisation (IVF)
In broad terms the IVF procedure consists
of recovering eggs from ovary, fertilising
them with sperm and transferring the resulting
embryos into the uterus. In this procedure
the sperm and eggs are simply cultured together
overnight.

Intracytoplasmic
Sperm Injection (ICSI)
Many patients undergoing IVF fail to achieve
fertilisation. Often these cases are due
to male infertility where the sperm is unable
to penetrate the outer shell of the egg.
The micromanipulation techniques we use
help men with severe sperm impairments.
By directly injecting sperm into the cytoplasm
of the egg, the natural barriers that the
sperm encounters are bypassed. |
 |
ICSI is indicated
for the following reasons:
- Suboptimal semen parameters (low sperm
count, poor motility, low normal sperm
morphology, etc.).
- Previous failed fertilisation in IVF.
- Following surgical sperm retrieval.

Sperm
and Embryo Donation
Gamete donation raises many complex issues and couples
contemplating such treatments psychological implications
will be explored fully. Sperm donors are matched
as closely as possible for physical characteristics
and blood group to the male partner of women receiving
the donor sperm. Donor sperm can be used for donor
insemination and for IVF.
Eggs are recovered from the donor and the sperm
from the recipient’s partner is used to inseminate
the eggs in the laboratory either by IVF or ICSI.
The resulting embryos are transferred two or three
days later to the recipient’s uterus.
Egg donation is indicated for the following reasons:
- Premature menopause.
- Genetic abnormality.
- Poor ovarian response to hormonal stimulation.
- Absence of ovaries.
- Ovarian failure due to chemotherapy/radiotherapy.

Sperm Cryopreservation
Most semen samples can be frozen for long periods
without loss of fertility. Sperm cryopreservation
is indicated for the following reasons:
- Prior to radio/chemotherapy
- Patient’s with spinal injury, following
electro-ejaculation or surgical sperm retrieval
- Surplus sperm obtained following routine surgical
sperm retrieval.
- Prior to Vasectomy.
- Patients with very low sperm numbers may freeze
repeated samples.

Embryo
Cryopreservation
Following initial treatment a maximum of
three embryos are transferred. Depending
on the quality of the spare embryos, they
may be frozen for transfer at a later date.
This center has facilities for freezing
using latest technology from Crygologic
of Australia. We are the sole center that
offers Cryopreservation in the capital.

to complex sperm function tests. It also provides
all sort of hormonal assays, for both males and
females.

Ultrasonography
and Interventional Sonography
We are equipped with the latest scan machine,
both trans abdominal and trans vaginal probes.
Colour Doppler is also available. With the
help of ultrasound, operative procedures
are carried out such as foetal reduction,
aspiration of cysts etc.

Laparoscopic
and Hysteroscopic Surgery
Laparoscopic surgery which is minimally
invasive, is the appropriate method of treatment
especially infertility. This center is equipped
with most modern gadgets required for keyhole
surgery. The patient can be discharged within
24 hours and can try for conception immediately.
The other advantage is the time taken to
resume the normal activities is less than
2 weeks when compared to 4.6 weeks rest
advised in Laparotomy. The Laparoscopic
surgery would cost the same as open surgery.
Hysteroscopic procedures for viewing inside
the uterus and surgery like excision of
septae, cannulation of tubes, removal of
submucous fibroids and resection of the
endometrium in patients who want to retain
the uterus in severe bleeding (DUB).
