INFERTILITY : THE ISSUE


About 15 % of couples world wide have difficulty in achieving pregnancy. There are many reasons leading to infertility, 40% are related to men and 40% to women, 10% related to both, and 10% due to unexplained causes.

Male Infertility is one of the largest causes of subfertility. There is a declining trend for seminal parameters throughout the world. The stress associated in this highly competitive world, environmental pollutants, smoking, vancocoele and sexually transmitted diseases are the causative factors.

Female factors
The reasons of female infertility may be due to failure to ovulate, obstruction to fallopian tube, infections such as pelvic inflammatory disease due to tuberculosis or sexually transmitted diseases. There is a high incidence of endometriosis in Kerala. In a study conducted in Fertility Centre for past 4 years, endometriosis has been found to be the cause in 28% of female causes of infertility.

The cause of high incidence of endometriosis is not clear but it may be due to environmental pollutants like Dioxin.

Laparoscopic Surgery which is minimally invasive surgery is the appropriate method of treatment. This centre is equipped with most modern gadgets required for key hole surgery. The patient can be discharged within 24 hours and can try for conception immediately. The other advantage is that the time taken to resume normal activities is less than 2 weeks when compared to 4-6 weeks rest advised in Laparotomy. The cost of Laparoscopic Surgery is equal to open method.

Ovulation disorders

This accounts for 30% of female factors. During a four year study period from 1997-2001 it was 1997-2001 it was found that Polycystic ovary was the leading causative factor for infertility in 37% of cases. These patients are obese and have irregular menstrual cylces and may be associated with increased hair growth.

Assistance may be given to achieve pregnancy by proper medications, measures taken to reduce weight and monitoring ovulation by Ultrasound Doppler Study and estimation of hormones. The highly sophisticated Toshiba Colour Doppler ultrasound scanner at the center benefits these patients. The estimation of hormones is by highly sensitive Elisa method. To optimise the chances for conception an ovarian stimulation protocol is followed which aid growth of follicles and bring about ovulation.

Assisted Conception Intrauterine insemination

When conception does not occur normally, Intrauterine Insemination is carried out which involve drugs for follicular stimulation and ovulation induction. When ovulation occurs the sperm sample is carefully washed and is placed high in the uterus. The treatment is carried out for 4-5 cycles.

In Vitro Fertilisation
This procedure consists of recovering eggs from the ovary, fertilising them with sperm and transferring the resulting embryos into the uterus. This is indicated in tubal damage, severe ovulatory problems, minor sperm problems and unexplained infertility. Success rate in this case is around 20-25%.

Intracytoplasmic injection
A number of patients undergoing IVF fail to achieve fertilization where the sperm is unable to penetrate the outer shell of the egg. ICSI is used to help men with severe sperm impairment. By directly injecting the sperm into the cytoplasm of the egg the natural barriers that the sperm encounters are bypassed. ICSI is indicated when the semen parameters are suboptimal, previous failed fertilisation in IVF and unexplained infertility. The success is around 40%.

The KJK Fertility Centre is the only one of its kind which offers ICSI procedure in Trivandrum.

Cryopreservation
(Freezing and storage)
Sperm Cryopresevation

Most of semen samples can be frozen for long periods and thawed without loss of fertility. The legal limit in UK is 10 years. Sperm Cryopreservation is indicated prior to radio/chemotherapy prior to vasectomy, husbands working in gulf countries and donor insemination.


Embryo Cryopreservation
Embryo Cryopreservation is indicated depending on the quality of spare embryos that may be frozen for transfer at a later date. The replacement of frozen embryos can be carried out after treatment with hormone replacement therapy.

This centre has facilities for sperm and embryo freezing using latest technology of Cryologic from Australia. The only centre which offers Cryopreservation in the capital.

Other services provided
Laparoscopic Surgery for fibroids of uterus, cysts in the ovary, microtubal anastomosis for recanalisation. Gynaec cases- Conservative surgery as well as Hysterectomy-Conventional and Laparoscopic assisted.

Hysteroscopic procedures for viewing inside the uterus and surgery like incision of septae, cannulation of tubes, removal of submucous fibroids and resection of the endometrium in patients who want to retain the uterus in severe bleeding (DUB) Interventional Sonography -with help of ultrasound, operative procedures are carried out such as foetal reduction, aspiration of cysts.

FOGSI (Federation of Obstetric and Gynaecologic societies of India has recognised this Fertility Centre for training in Laparoscopy.
